Care Coordinator

A twelve-month contract covering maternity leave, coordinating domiciliary care for around 180 service users across north Leeds.

What you will do

• Build and maintain the weekly rota for 40 care workers, and hold it together when it breaks at 7am.
• Be the first point of contact for families. Many of these calls are difficult.
• Keep care plans current and audit-ready.
• Work with the clinical lead on escalations.

What we are looking for

• Experience coordinating care, or strong scheduling experience in another high-stakes setting.
• NVQ Level 3 in Health and Social Care, or willingness to work towards it. We will fund it.
• An enhanced DBS check, which we will arrange and pay for.

This role is demanding and the on-call rota is one week in four, paid at an additional rate. We would rather tell you now.
